Case Management Care:

Is the process of coordinating all the clients individual needs that encompass all their health care providers, facilities, therapies, medications, dietary needs, etc., and integrate them seamlessly to facilitate the optimal level of care thereby giving the client the best chance of recovery. This is especially advantageous when clients are transient and not knowledgeable regarding all the practice specialties required in the event of an unforeseen medical emergency.

Case management nurses work in many environments including hospitals, home health agencies, long term and assisted living facilities, government agencies and in local communities. They work closely with physicians, nurses, physical therapists, nutritionists, social workers and a wide range of health care providers. Case Managers work as the central hub for the client’s special needs by directing inquiries, gathering medical records, scheduling meetings and consults, as well as, referring services to specialty areas of health care to fulfill our client’s needs.

At Home Health Care works hard to assure that everyone involved in the client’s care is updated on their current health care status. We believe that by working together we help to strengthen the client’s support systems which allows them to enjoy what’s important in their lives like family and daily activities.

This way you don’t have to worry about the logistics of your health care because the Case Manager will gather all the information and lay out the best options for you and your family to choose what is best for you!
